Presumo che (patient_id, adherence_date, scheduled_time)
è unico nella tua tabella, il che significa che un paziente può prenotare una volta per "slot" e data.

La logica funziona così:
- se riconosciuto_at è nullo, aggreghiamo 0 (tramite nvl2)
- se riconosciuto_at non null quindi aggreghiamo 1 (tramite nvl2)
- se non ci sono record per questa fascia oraria, aggreghiamo null (dal caso in cui ... fallito)
