Ecco come ho fatto.
Ho riutilizzato un campo inutilizzato in patient tabella per contrassegnare i pazienti non duplicati (N), 1° di duplicati (X) e altri pazienti duplicati (Y). Puoi anche aggiungere una colonna per questo (e rilasciarla dopo l'uso).

C'è sicuramente un modo più breve, con un SQL più intelligente (complicato?), ma personalmente preferisco il modo semplice. Questo mi permette anche di controllare che ogni passaggio stia facendo ciò che mi aspetto.
