Per prima cosa dovresti usare print_r($_FILES) per eseguire il debug e vedere cosa contiene. :
il tuo uploads.php sarebbe simile a:

$target = $target . basename( $_FILES['Filename']['name']);

$Filename=basename( $_FILES['Filename']['name']);
$Description=$_POST['Description'];

MODIFICA: Poiché questo è un vecchio post, attualmente si consiglia vivamente di utilizzare mysqli o pdo invece mysql_ funziona in php
