Come dare alla posizione zero dello spinner un valore di richiesta?

Stai ricevendo i dati da db in al Lista di array. Quindi puoi fare quanto segue

al.add(0, "YOUR MESSAGE");    

Aggiunge la stringa IL TUO MESSAGGIO a 0th indice.

Inserts the specified element at the specified position in this list. Shifts the element currently at that position (if any) and any subsequent elements to the right (adds one to their indices).

Dopodiché, passa la lista ad arrayadapter

ArrayAdapter<String> aa1 = new ArrayAdapter<String>(
            getApplicationContext(), android.R.layout.simple_spinner_item,
            al);

    spn.setAdapter(aa1);
